Demographic Forms

Please complete the demographic form your child brings home this week.  This is a requirement every year to ensure we have your correct information and address in our system.  If you have completed one online or have already completed a paper one, you do not need to compete another one.  Please return the form as soon as possible as we are required to have the information updated in our system by October 31, 2024.
